EMPLOYEE NOTIFICATION, PRE-EMPLOYMENT AND RELEASE

Drug Testing Consent Form

I have applied for employment with Hotel Valley Ho. As a condition for my application being considered, I understand and agree to undergo substance screening should I be offered a position with Hotel Valley Ho.

I understand that employees may be selected at random for drug testing at any interval determined by the company. I understand that if I am chosen to undergo substance screening, I have 24 hours to undergo screening at the Hotel's authorized facility. I understand that if I do not undergo substance screening within the 24 hours I am choosing to voluntarily terminate my employment with Hotel Valley Ho.

I understand that if I should test positive on the drug screen, indicating the presence of a prohibited drug or substance, I further understand I will be terminated immediately.

I hereby authorize any physician, laboratory, hospital or medical professional retained by Hotel Valley Ho for screening purposes to conduct such screening and to provide the results to Hotel Valley Ho, and I release Hotel Valley Ho and any person affiliated with Hotel Valley Ho and any such institution or person conducting the screening, from liability.
